Health & Home Solutions will not share your name, phone number, or any other information about you. We do not do telemarketing. We ask only for your phone number in the event that there is a problem with your order. We will send to you periodic mailings with information on new products, special services or upcoming events to the postal address provided on-line. If you do not wish to receive such mailings, please contact us and we will be sure your name is removed from the list we use internally.

Credit Card Information
Because your credit card security is a high priority, we have taken numerous steps to ensure that your payment information is processed confidentially, accurately and securely. Health & Home Solutions uses Secure Sockets Layer (SSL), an encryption technology that protects your credit card information. SSL encrypts all ordering information so no one can read it over the internet.
As an additional layer of security for orders placed on the internet, we do not keep records of your credit card number or expiration dates on file. This information is passed directly through to our credit card processing company during the ordering process.

privacy and security implications of HIPAA. Health & Home Solutions
understands the values embodied in these rules to protect the privacy
and security of individually identifiable healthcare information. We

NURSE ASSISTANCE:
BLADDER HEALTH INFORMATION, PRODUCT SELECTION & USAGE
Health & Home Solutions registered nurse health educators are available for product consultations and general bladder health information to help individuals effectively manage their incontinence within the home or social model of care.
Health & Home Solutions does not provide medical advice, diagnosis or treatment for incontinence. However, the health information that you provide regarding your incontinence condition during a conversation with our nurses may be subject to HIPAA privacy policies. Some of the uses and disclosure of information shared with Health & Home Solutions requires your specific authorization. Such activities may include simple acts as receiving or sending a fax for your incontinent supply order or discussing home management alternatives or self-care interventions with a healthcare professional, family member or friend that you indicated is involved in your care.
Health & Home Solutions, as a preferred provider of services to physician offices or assisted living communities, may require your approval to share your incontinence supply order to ensure Health & Home Solutions professional customers meet with their HIPAA compliance. Such situations may include being able to receive a fax from your physician for state Medicaid reimbursement or communicating with an assisted living community where you are presently residing.
RIGHT TO LIMIT USES AND DISCLOSURES
You have the right to limit how Health & Home Solutions uses and discloses your information. However, you may not limit the uses and disclosures that Health & Home Solutions are legally required or allowed to make. The following is a select list of examples of where you may limit how Health & Home Solutions utilizes your health information:
- A. If you choose to sign an authorization, you may later revoke that authorization in writing to stop any future uses, to the extent that Health & Home Solutions has not taken any action relying upon that authorization.
- B. You have the right to look at or receive copies of Health & Home Solutions uses and disclosures. There may be charges for copies.
