Bladder Activity Log

An accurate Bladder Activity Log gives your doctor a clearer picture of your unique situation. Try to keep your diary for at least four days before you see your doctor, and then take it with you to your appointment.

Bladder Activity Log instructions:

  1. Record the date and time of day when you use the toilet or have leakage.
  2. Record the amount of fluid you have consumed since you last voided.
  3. Indicate the amount voided (small, medium, large) in the “Toilet” column.
  4. Indicate the amount of leakage (none/damp/wet) in the “Leakage” column.
  5. Note your activity at the time of any leakage
  6. Indicate if you had an “urge” to go.
